Personal details: Your name, age, address, telephone numbers, email address, and the name of your general medical practitioner.
Medical and dental history: Your past and current medical and dental conditions.
Clinical records: Radiographs (X-rays), clinical photographs, and study models.
Treatment information: Details of the treatment we have provided or propose to provide, including associated costs.
Incident records: Notes of conversations or incidents that require documentation.
Consent records: Documentation of your consent to treatment.
Correspondence: Communication with other healthcare professionals, such as referrals to specialists.

We maintain comprehensive and accurate personal data about our patients to ensure safe and appropriate care.

How We Process Your Personal Data

1- Retention of Records:

We will retain your dental and medical records while you are an active patient at our practice.
If you cease to be a patient, we will continue to hold your records for at least 10 years (or, in the case of children, until they reach the age of 23), as required by Swedish healthcare regulations.

2- Storage and Security:

Your personal data is stored in our secure computer system and/or in a manual filing system.
Only authorized members of staff have access to this information.
Our computer system has secure audit trails, and we back up data daily to ensure its integrity.

Disclosure of Personal Data

To provide proper and safe care, we may need to share your personal information with:

Your general medical practitioner.
Hospitals or community dental services.
Other healthcare professionals involved in your care.
Dental insurance companies or private dental schemes of which you are a member.

Disclosure will occur on a “need-to-know” basis, meaning only the necessary information will be shared with authorized individuals or organizations. Recipients are also bound by strict confidentiality rules.

In very limited circumstances, or when required by law or a court order, personal data may be disclosed to a third party not involved in your care. In all other situations, disclosure will only occur with your specific consent. Where possible, you will be informed of any such requests for disclosure.

Your Rights

1- Access to Your Data:

You have the right to access the personal data we hold about you and to receive a copy.
To request access, please submit a written request to us.
We may charge a reasonable fee for providing copies of your records. For example:
– Dental records: A fee of 100 SEK may apply.
– X-rays: An additional fee of 400 SEK may apply.
We will provide the requested information within 30 days of receiving your request and payment.

2- Transfer of Records:
– If you move to another dental or medical practice, we may (at our discretion) transfer your original X-rays and provide copies of your records directly to the new practice, free of charge, upon receipt of a written request from them.

3- Objections to Processing:
– If you do not wish your personal data to be used or disclosed as described in this policy, please discuss this with your dentist.
– You have the right to object, but please note that this may affect our ability to provide you with the necessary care.
